The North West London Integration Toolkit is intended to support commissioners, providers and communities across health- and social-care as they work toward the shared vision of integrated care. It may be useful as the basis for discussions and will help in the completion of business cases for local initiatives.
The toolkit will evolve. Comments and contributions are welcomed and the toolkit will be updated regularly in the light of these.
The content of the toolkit has been co-designed and tested with a wide range of stakeholders, including people who use services, carers, commissioners, local authorities, acute and primary care providers, mental-health and community trusts, social-care and voluntary sector providers, public health and the Academic Health Science Network for North West London.
More detail is contained in the Supporting Materials, which include a collection of the detailed conversations held in the working groups during the co-design phase.
