These tools are designed to support and proactively engage with the growing number of individuals in North West London living with and at risk of LTCs condition. In NW London there are over 350,000 people living with hypertension, 175,000 with diabetes, and 215,000 with NDH (pre-diabetes), many from ethnic minority and deprived populations. (NWL ICB Data, 2024).

The management of LTCs in NW London has significantly transformed in recent years, with the development of this digital platform- MyHealthLondon and KDS has revolutionised care for thousands of people living with and at risk of LTCs by providing a single point of access for all necessary information to support healthier lifestyles, and empower them to manage of their conditions.

- 51250 engagements from diverse population demonstrated by their ethnicity
